Digital remote work reporting form now available for use 

The digital form will replace the Excel logs initially shared with employees.

The Office of Human Resources has developed a digital reporting form for employees to report the use of remote work, as required by state and University policy.

Employees must log into their Clemson University Microsoft 365 account to use the Microsoft Form. This will link the necessary employee information to the submitted responses.

Once submitted, a copy of the form will be sent to the supervisor for recordkeeping. This form is for reporting purposes only. Employees must discuss any remote work arrangements or schedule modifications with their supervisor before submitting this form.

Employees should submit one form for each day.

Previous Excel Reporting Forms

Employees should input the information recorded in the Excel Remote Work logs for October and November into this Microsoft Form.

Use of the Excel logs is no longer necessary.

Reporting Requirements 

Clemson University is required to document and report all use of remote work.

Employees enrolled in the University Remote Work program document their regular remote work schedule in their Remote Worker Agreement form.

Those enrolled in the program should use the digital form only to document changes to their approved remote work schedule, including:

  • Working remotely, when an employee was scheduled to be in the office.
  • Working in the office, when an employee was scheduled to work remotely.

Those not enrolled in the University Remote Work program must report any use of remote work.
