For employees who opted for the electronic version of the 1095-C Tax Form, the forms are now available online and may be downloaded.
Select the View Form 1095-C link located on the left side of your ESS Tax Forms webpage to download the form.
If you have not consented to the electronic version, your 1095-C was mailed to your home address on March 1, 2022.
If you have trouble accessing your 1095-C online, make sure your browser allows pop-ups. For further assistance, contact the CCIT Support Center.
